******REFILLS******for prescriptions including controlled substances requires 48 to 72 hours. An informed consent for all controlled substances, which include narcotics pain management and medications such as Xanax, Ambien, Diazepam, etc. is required as per NYS and federal regulations. Please schedule an appointment every 3 months to be in compliance with the state of New York for these prescriptions and the contract you signed. If you fail to keep your appointment every 3 months, refills will NOT be dispensed.

We will be sending reminders to patients for some of the following services if you are due for them, Mammograms, PSA Blood tests, Blood Pressure follow up, Pulmonary Function Tests, Diabetic Screening tests, Immunizations and other health maintenance services.

If you are admitted to area hospital or facility a provider/hospitalist will be assigned to you during your stay.
Please ask that ALL of your hospital records are faxed to Dr. Sofat upon your discharge.
Also, schedule a follow up within 1-2 weeks of your discharge, and bring discharge paperwork and all of your medications with you to your follow up appointment after hospitalization.
